Modifier 95 Defined: Synchronous telemedicine service rendered via real-time interactive audio and video telecommunications system.

During the COVID-19 Public Health Emergency, modifier 95 should be used when a real-time interaction between a provider at a distant site and a patient at an originating site occurs via an audio and video telecom system. It should be appended to all services as indicated on CMS’ List of Telehealth Services. This list consists of CMS’ approved telehealth services and are the only codes modifier 95 should be appended to.

Other services such as standard telephone (audio only) or e-visits (services provided via online patient portals or email) are not defined as telehealth services because the audio and video component was not utilized. Therefore, modifier 95 should not be appended to audio video consultations.
